Assisted Living:

Waltonwood  Twelve Oaks is dedicated to improving the quality of life for seniors in the privacy of their own residence. Amenities include 1 & 2- bedroom apartments with a kitchenette, 3 nutritious meals, hydration program, housekeeping/laundry services, courtesy transportation, 24-hour on-site care, personalized care plans, medication management program and social activities. Our licensed staff offers caring, professional assistance around the clock and we are able to accommodate ever changing needs.

Memory Care:

Waltonwood Twelve Oaks’ goal is to restore a resident’s sense of self and help them overcome the fearful anxiety of memory decline. We provide encouragement and support while helping residents make their own decisions. Amenities include three nutritious meals, assistance with daily living activities, housekeeping and laundry and 24-hour on-site care.
