Altenheim at Home was established with the belief that each individual regardless of age, race, religious beliefs, national origin, sex, sexual preference, handicap, or economic status is entitled to maximize his or her optimum level of health status towards more complete physical, mental and social well-being.  As a result, the mission of Altenheim at Home is to provide high quality, comprehensive home care services through a coordinated plan of care to the ill, aging and disabled residents of Ohio’s Cuyahoga, Medina and Lorain Counties. 

Altenheim at Home provides personal care and related assigned services to home care clients in their place of residence, nursing homes and inpatient units in accordance with an established care plan and under supervision of the Nursing Care Manager.

The following services can be provided:

Personal care activities including but not limited to hygiene, mobility and elimination

Light housekeeping and meal preparation.

Monitors fluid and nutrition intake

Vital signs and basic health assessments

Maintenance of records and reports

Transfers, ambulation, normal range of motion and positioning techniques

Monitors the need for intervention

Services are available 24 hours a day seven days a week.
